Our skilled nursing professionals will help you navigate through your disease, manage your care in your home, and help you become more confident in caring for yourself and/or a loved one. Our nurses will communicate with your physicians and other team members, to ensure you are getting the best care possible.

Work in collaboration with each other, your physician, you, and your family/caregivers, to ensure you are getting the best possible outcomes. We know we cannot stay with you forever, so supporting you, teaching you, and helping you regain your strength and confidence is necessary in helping you reach your maximum potential.

 We come to you, saving you time traveling and sitting in a waiting room. We help you reduce your hospitalizations and trips to the ER, again saving your valuable time. We help you to manage your disease process independently (0r with the help of your family), allowing you to spend quality time with those who mean the most.

We come to where you are, in your home, your family’s home, assisted living or group home facility. Where you feel most comfortable. This allows us to see firsthand what barriers you might be facing and work with you to come up with solutions.

You guide your plan of care, the clinicians work with you to develop a plan of care that supports your goals. We can’t do it without your input!

We work with you, your family and your caregivers to understand your disease, what makes your symptoms worse, what makes them better, and what are the “red flags” that alert you that intervention is needed right away to avoid a trip to the ER. We review all of your medications, assess for side effects, and interactions, and ensure you are taking medications exactly how your physician orders them. We perform skilled procedures, assess their appropriateness, and monitor for signs and symptoms of infection. We teach and educate you, your family, and your caregivers on safely performing procedures. We work with you to make you stronger and safer when ambulating, transferring, and performing activities of daily living (bathing, dressing, cooking). Together, we develop the tools, resources, and knowledge necessary to allow you to manage on your own once it is time for us to leave.

Our Services

Our licensed and skilled therapists will work with you to get back to your prior level of functioning, maintain your current level of functioning, regain independence, assist with pain management, and safety in the home, and reduce falls. Our therapists will perform an evaluation and develop a personalized plan of care that supports your goals.

Understanding why you are being prescribed them, how they affect your body, when and how to take them, possible side effects or contraindications, and answer any questions you might have.

Whether it is a complicated or simple wound, wound vac, pressure ulcer, diabetic ulcer, cellulitis, skin tares, or a surgical wound. Our nurses are well-trained to monitor, observe, and assess the wound for appropriate wound care, and collaborate with your physician to ensure your wound is getting the best wound care possible. Our nurses will teach you and/or a family member/caregiver in how to manage that wound care, and what signs or symptoms of infection to watch for, and we are always available to assist you. We want you to be confident in caring for yourself.

Our nurses will observe, assess, and monitor the effectiveness of your IV therapy. We will help you, a family member, or a caregiver become comfortable in performing the infusion in the comfort of your own home. We will perform any sterile procedures where you are most comfortable. Together, we will help you get back to feeling like yourself.

Will assist in transfer safety, walking, navigating steps, fall safety, strengthening, balance, and pain management.
Will assist you in improving your activities of daily living: bathing, dressing, eating, and strengthening.
Will assist you in improving your ability to speak, swallow, and communicate.
Will assist you and your family in obtaining resources in the community, help you navigate your benefits, and assist you in moving to a higher level of care, such as assisted living, a group home, or a skilled nursing facility. They also offer short-term counseling.
Will assist you during a time when you need that extra assistance. While you are working with your occupational therapist in independent safe bathing, our home health aides will come in and assist you with safe bathing. Their care is short-term, as again our goal is to get you as independent as possible. We want you to be safe, even after we discharge you.
