Skilled Home Health care is a Medicare regulated division of health care. To qualify for services, patients must meet these 2 criteria: The patient must be homebound (which simply means they need assistance to leave the home), and they must have medical necessity that would require the specific skills of a licensed professional healthcare provider (nursing, therapy). As a veteran, when using your veteran insurance, these requirements do not apply to you. If you have any questions please reach out to us for further assistance.

Your physician or a hospital physician can send a referral to us to request services. Skilled Home Health care can only be ordered by a licensed physician, NP or PA. Please call us if you have questions or need any assistance. We are happy to reach out to your physician for you.
A Skilled Home care episode is 60 days, as long as there is a skilled need and you continue to be “homebound”, you qualify for services. A couple weeks prior to the end of your episode your clinicians will evaluate your progress towards goals. If there is still a skilled need then they will collaborate with your physician and discuss extending your care. Skilled services are short term and intermittent. We are not a long term care option.
We are glad you asked! We cannot do this without you and your involvement. We even encourage your family and caregivers to be involved. The most successful outcomes come from collaboration. This includes the nurse, therapist, aide, physician, patient, family, caregivers, social worker, hospital, specialist, and anyone else involved in your care. We are a team. We need your input, your goals. You are the captain of this ship, we are the skilled professionals that help you navigate it.
Yes, as long as you have a skilled need and meet the guidelines for being “homebound”. If you have veteran insurance then as long as your VA physician continues to certify you, you can have skilled services indefinitely. We are happy to speak with you to discuss the qualifications and to see if you meet the eligibility requirements.
Private caregivers are typically in the home for multiple hours at a time and usually come daily or several times a week. These caregivers help you clean, cook, shop, take you to appointments, remind you to take your medications, and help you to bath. Skilled home care is meant to help you get back to your prior level of function. It is usually ordered by the hospital or your physician upon a new diagnosis, surgery, injury, or decline in health. We help you regain your independence, function, and manage your disease, so you can continue living your life to it’s fullest.

Once we receive the complete referral from your physician, we will have someone out to see you within 48hrs. We will contact you prior to your first visit to welcome you and verify your information. You will then be contacted by the clinician (nurse or therapist) to schedule your initial assessment.

Most insurance companies cover home health care services. Straight Medicare, Medicaid, and VA insurance does not require a copay or deductible. Other commercial insurance may have copay or require you to meet a deductible. Don’t worry, we will help you navigate your coverage and inform you prior to providing any services what payment you may be responsible for, if any.
